[hle] Fix crash on MK8D and SM3DW LDN ( #4417 )
...
Crash is due to SIGILL due to mismatched N and span<> ctor().
Also some astray nullptr references.

[core/file_sys] implement IApplicationFunctions::GetPseudoDeviceId; common PatchManager::GetNACP for base|updates ( #4159 )
...
a bit of refactoring so there's less code duplication...
probably a good idea to make it a lambda anyways
maybe this is why QLauncher didn't have proper DLCs and updates?
either way "GetPseudoDeviceId" is now implemented (still stubbed) with a proper hash instead of just 0

This formats all copyright comments according to SPDX formatting guidelines.
Additionally, this resolves the remaining GPLv2 only licensed files by relicensing them to GPLv2.0-or-later.

Should the input string not meet the above requirements, an assert will be triggered and an invalid UUID is set instead.

This allows for easily converting a hex string into a Common::UUID, which is backed by a 128 bit unsigned integer.

Since the Mii database uses UUIDs very similar to the Accounts database, it makes no sense to not share code between them.
